中醫治療妊娠皮膚斑疹病例報告

A Case Report on Treating Macular Rash in Pregnancy with Chinese Medicine

摘要

妊娠期間因體內荷爾蒙的變化，皮膚會出現相應的生理改變及病理表現，孕期婦女常見的皮膚症狀，如搔癢或皮疹，有懷孕特異性表現，亦有原本皮膚問題隨懷孕而加重，病灶表現多樣，鑑別診斷亦多。本案例為一41歲女性，於第三孕期開始出現皮膚搔癢，發無定處，初起並無皮疹，但至分娩後開始，搔癢情形加重，伴隨腿部出現散發的斑塊，斑塊逐漸浸淫成片，刺癢難忍。陸子賢所著《六因條辨》曰：「斑為陽明熱毒，疹為太陰風熱，總屬溫熱所化」，與此患者熱盛的表現相符，治以調胃承氣湯及黃連解毒湯為主方，斑塊大幅緩解。

Pregnancy is associated with a variety of normal physiological skin changes as well as excerbation of pre-existing skin diseases or development of new pregnancy-specific dermatoses due to hormonal fluctuations. The various skin conditions and lesions may complicate the process of differential diagnosis. In this clinical case, a 41-year-old woman experienced itching, without rashes during the third trimester of pregnancy. The symptom after delivery grew worse instead of resolving. In addition to intense itching, macules appeared on legs gradually enlarged and coalesced to form patches. We treated the patient with Tiao Wei Cheng Qi Tang (Powder) and Huang Lian Jie Du Tang (Powder) as Lu Zixian's "Six Causes Discrimination" in Qing Dynasty mentioned that "macules are caused by Yangming heat". Skin condition improved significantly within one month after intervention, thus we report the case.
