牙周器械處理後的牙根面變化─掃描式電子顯微鏡相之評估

摘要


牙結石刮除及牙根面刮平術是牙周病治療的基本方法之一。 其目的不但可以除去牙齒表面之菌斑、牙結石及沈積物,也可提供較扁平滑的牙根表面進而使物質不容易再附著、沈積。牙周器械在牙根面所造成的正面與負面效果,研究報告結果顯示部不盡相同。本研究的目的乃是利用掃描式電子顯微鏡觀察各種不同牙周器械及方法處理後的牙根面變化。所使用的牙周器械及方法包括:超音波洗牙機、Gracey牙刮、磨光杯沾研磨劑磨光及氣動噴砂式潔牙機。分別及交替使用於因罹患嚴重牙周病而拔除的牙齒之牙根面,然後以電子顯微鏡觀察。結果發現: (1)在無環填因素的限制下,以牙刮做牙根面的刮平,將可獲得較為均勻、平坦的牙根面且牙結石的殘存量較少。 (2)沒有經過刮平的牙根面,雖然利用磨光術來處理,並不能連到去除牙顎面沈積物的目的;但經過器械刮平後再磨光,則可得到較為平滑的牙根面。(3)氣動噴砂式潔牙機在牙根面的清除效果與橡皮杯沾研磨劑處理相近;但其研磨顆拉附於牙根面上不易沖洗掉,若嘗試使用於牙眼下的牙根表面則有待進一步之探討。

Periodontitis involved teeth which were due to be extracted were selected for the study after extraction, they were treated with single or combined instrumentation: Cavitron uitrasonic scaler, Gracey curette, rubber cup loaded with prophy-paste as prophy-jet, washed, fixed and stored briefly before critical point drying and preparation for examination in the scanning electron mIcroscope. All surfaces are rough when observed at an ultrastructural level. A small amounts of calculus were found over some areas which can't be detected by probe exploring. Left less residual calculus, Gracey curettes were superior over the ultrasonic instrument, in the accomplishment of root smooth and removal of calculus. Although polish procedure can't remove calculus, but can smooth the scratching outlines produced by curettes or ultrasonic sealer of the root surfaces. Prophy-Jet can achieve the same polish effect on root surface like rubber cup loaded with phophy-paste but the residual particles over the root surfaces can not be rinsed out easily by nonnal saline. The effects of the residual particles of the abrasive powder on the periodontal tissues or on the systemic of patients with sodium-associated hypertension warrant more future research.
