CSC－套疊冠在補綴學上的臨床應用
摘要
csc套疊冠是一種依靠選擇性固位牙及無 牙區齒槽骨或粘膜支持的活動性補綴裝置,這 種csc套疊冠的補綴裝置能經由(1)長錐形(the conical ta per)的外形設計, (2)套疊冠的波特 間隙(clearance for rotation)' (3)橋基牙的夾板被固定及縮小冠根比率設計(Splinting and a shortened level)等以上三種生物力學原理( biomechanic priuciples)的合併應用而提供一 種在生物力學土可以接受的連接體(biologically acceptable linkage)。由於csc套疊冠的補 綴裝置具有改善固位效果(retention)、穩固的 垂直支持力(rigid vertical scpport)、冠根比 率之改善、補綴物的穩定性( stability)和固位 牙的本體感受性(proprioception)等特性,因 此能克服一般傳統性假牙及覆蓋式假牙的缺點。此外, csc套疊冠的補綴裝置,具有維持脆 弱的牙周患齒、牙菌斑控制容易、減少不正常的水平應力及破壞性扭力作用於牙周組織之減少、較美的外觀(與局部性假牙比較)、轉變全口假牙之容易性等多項優點,因此特別通用於進行性破壞之牙周病患者之全口復健。( oral rehabilitation) 總而言之， csc套疊冠補綴裝置,乃是依擴 物力學原理設計的合併應用;非但改善了傳 統性假牙與覆蓋式假牙的缺點,而且能提供更 舒適而健康的咀嚼能力。要注意的是通當的選 擇病例,明確的治療計劃,及正確機械與生物 力學設計的評估,是確信csc套疊冠補綴裝置 成功的不二法門。

The crown and sleeve-coping (CSC) is a removable prosthesis that is supported by selectively retained teeth and the underlying residual ridge or mucosa. This device provides a biologically acceptable linkage through the application of three biomechanic principles: 1) the conical taper, 2) clearance for rotation, 3) splinting and a shortened level. The CSC serves to overcome problems common conventional dentures and overdentures due to its significant advantages of improved retention, rigid vertical support, improvement of the crown-root ratio, and stability and proprioception. Additionally, the use of the CSC prosthesis, particularly, as a final periodontal prosthesis in advanced cases provides numberous advantages including maindestructive torque, along with less leveling forces on the periodontal support, destructive torque, along with less leveling forces on the periodontal support, and is more esthetic than a partial denture and is easily adapted to future conversion of the denture. In general, the. utiliza.tion of the CSC prosthesis will afford patients the means to retain their masticatory ability in comfort and health, without being dependent upon full dentures, In addition,the keypoints of case selection, patient evaluation and proper mechanical therapeutic measures will insure a beneficial result of CSC prosthesis on a predictable basis.
