心智化導向治療模式之經驗性團體初探

A Pilot Study on A Mentalization-informed Experiential Group

摘要


目的:本經驗性團體旨在訓練某醫學中心的諮商心理碩士實習生,目的在於熟悉心智化理論,並透過經驗性團體來體驗臨床技巧,以增進實務工作的能力。方法:本團體為一個短期非結構封閉式團體,由擔任臨床督導的精神科醫師擔任催化員,以較為主動的介入,促進護持(holding )與涵容(containing)成員彼此的反應,進而提供一個安全依附脈絡,坦然地探索、修正情感經驗,開發個人的心智化能力。結果:經由分析、彙整每次團體後每位成員的心得紀錄得知,成員在團體歷程中除探索自己的情感反應,並且學習重新表達;除可以增進自我覺察的能力,也能夠熟悉團體壓力下的個人感受。結論:當面對團體中的壓力成員可以有更高的耐受性。此外,由於本團體兼具教育性質,催化員在過程中適時詮釋解釋團體現象和教導相關理論依據,也如期地增進團體成員的心智化能力。

Objectives: the purpose of this experiential group was to provide clinical training for master degree counseling psychology interns in a medical center in Taipei. The goals of this training were to understand theory of mentalization based treatment (MBT) and learn basic intervention skills for group psychotherapy. Methods: this group is a short-term unstructured closed group, and the clinical supervisor psychiatrist served as the facilitator. To help members explore and mentalize emotional experience, facilitator provided a secure attachment context and made a holding and containing environment by using an active, authoritative role. Results: by analyzing every member's post-group records after each session, the following were revealed that members could not only explore their emotional experience, but also express in a new way. Also, they had better ability of self-awareness and more familiar with their responses under stressful group situations. Conclusion: in addition to increase members' distress tolerance ability, with facilitator's due time interpretations about group phenomena and instructions of theoretic knowledge, members' capability of mentalization were also improved as expected.
