The majority of intraparotid hemangiomas are found in children below the age of one year. They are very uncommon in adults. Early symptoms usually include acute swelling, with thrombophlebitis and associated vascular congestion. The differential diagnoses include benign mixed tumor and chronic sialoadenitis with stone and infection. In this paper, we report a 67-year-old female patient with a cavernous hemangioma within the parotid gland. The patient noticed a painless, 2 x 3 cm nodule over the right preauricular area, Total parotidectomy and hemangioma excision with careful hemostasis were performed. The facial nerve was spared. The literature on hemangioma in adults is also reviewed. Though very rare in adults, cavernous hemangioma should be considered in the differential diagnosis of parotid lesions.
