頭部外傷後之外耳道膽脂瘤─病例報告
Post-Traumatic Cholesteatoma of the External Auditory Canal - Case Report
摘要
頭部外傷後引起的外耳道膽脂瘤為一罕見的疾病，常在顳骨骨折之後，經數年之久才被發現，最久的有長達24年之報告。目前認為由扁平上皮經軟組織缺損處和骨折處誤植入而形成。和平醫院耳鼻喉科於1995年6月經歷一17歲男性，在1994年8月因車禍造成左側顳骨骨折併急性硬腦膜下出血和右側顏面骨骨折，接受顱骨鑿開術。約10個月後，發生雙耳聽障及左耳持續性耳鳴及眩暈，聽力檢查呈現氣骨導差距58 dB故施予左側鼓室探查術，術中證實為左側外傷後之外耳道膽脂瘤，逐予移除並進行聽小骨重建，術後聽力幾乎恢復正常；目前門診追蹤下，並無復發之跡象。

In June 1995 we experienced a 17 year old male patient who visited our OPD with chief complaints of bilateral hearing impairment, left tinnitus and vertigo. Local findings revealed left external ear canal stenosis and the audiogram showed bilateral conductive hearing loss. The patient received a craniotomy due to head injury in Aug. 1994. Exploratory tympanotomy left ear, under the impression of tuaumatic ossiculopathy was performed. During the operation, huge cholesteatoma was found in the external auditory canal which extended to mastoid cavity. External ear canal plasty and modified radical mastoidectomy was followed and the cholesteatoma was removed. The patient tolerated the procedure well. And the patient received regular follow up in our OPD without evidence of recurrence still now.
