摘要
病人四歲因摩托車車禍，左眼眶部份受到機車手把直接穿刺，致使左邊視神經完全撕脫，以致左邊視力完全失明。視神經藏在眼窩內，受到良好的保護。一般是鮮有機會受傷，但如果受到外力直接或間接的撞擊，仍然會致使視神經斷裂或受損。臨床上視神經完全斷裂是非常少見的。雖然無法治癒，但站在急診的第一線醫師，對於眼部附近的受傷，必須有足夠的警覺心，才能正確診斷視神經受損程度，適時的會診和迅速的治療。

A 4-year-old boy sustained evulsion of optic nerve was reported. Evulsion or transection of the optic nerve is rare. The most common cause is following penetrating trauma of the orbit and its prognosis was poor. For facial injuries, the paitient's ocular status should be evaluated before, during and after the orbital surgery. Ocular injuries requires immediate diagnosis and a definitive treatment, if indicated.
