白色萎縮症是好發於中年女性下肢的反覆性及自發性潰瘍之疾病。其初期的病灶為紫斑性丘疹,進而形成表淺之潰瘍,而後形成不規則狀萎縮性疤痕。早期病灶之組織病理學檢查可見均句之纖維原狀物質,沈積於真皮層之血管中。其致病機轉尚無定論,而各種治療仍不能確實有效。本文報告一位43歲女性,因此疾病於五年內住院十一次並接受七次植皮手衛。本疾病之特性將為討論,並強調整形外科醫師之觀點。
Atrophie blanche is a disease of recurrent and spontaneous ulcerations of the lower leg, and it affects middle-aged women mostly. The primary lesions consist of purpuric papules which undergo superficial ulceration, followed by the development of irregular atrophic scars. Histopathologic examination of the early lesion for definite diagnosis showed deposition of a homogeneous fibrinoid material within blood vessels of dermis. The pathogenesis is obscure and the therapy is difficult. A 43-year-old female with such disease was admitted 11 times and received skin graft 7 times during these 5 years. The characteristics of this disease are discussed in this paper and the view from plastic surgeon will be emphasized.
