表淺性傷口之合成皮膚代用品治療

Synthetic Skin Substitutes for the Treatment of Partial Thickness Skin Wounds

摘要


表淺性傷口常發生於一般熱水燙傷,尤其是兒童。兒童燙傷傷口換藥往往必須住院上全身麻醉。減少換藥次數及住院天數是一個重要問題。另外,此類傷口如果沒有併發症延遲,應在2周內癒合,故病患均願能早日回到工作,但是凡士林紗及紗布包紮之傷口往往限制運動及影響日常生活,所以如何給予適當敷料是值得思考的。敷料種類眾多,不同的傷口亦有不同的特性,最好能針對傷口之特性給予適合的敷料處方。 本院30例病人接受七種合式皮膚代用品治療表淺性傷口,與凡士林紗及紗布敷料相較:其能減少換藥次數、減少住院日數及改善日常生活品質;而傷口癒合之時間相當。至於減少住院日數以早日回到工作及減少兒童上麻醉次數之價值則是不可衡量。

Partial thickness skin wounds often happen as a result of scald burn injury, especially in children. Dressing changes are difficult in children because they usually do not cooperative and must be aided with anesthesia. How to decrease anesthesin time and frequency is a important problem to be considered. Meanwhile, partial thickness skin wounds should be healed within two weeks if there are no complications. Most of the patients want to return to work earlier, but, the bulky gauze dressing limits their work and even daily life. To these wounds we must give a suitable dressing. There are different kinds of dressing materials, and how they are used depends on the nature of the wound. The effectiveness of dressing is influeneed by the appropriateness of its preseription at a particular wound site. There were thirty patients who received prescriptions of seven kinds of synthetic skin substitutes for the treatmenty of partial thickness skin wounds in our hospital. Compared with Sulfatulle-gauze dressings, the seven substitutes proved effective in reducing the frequency of dressing changes, decreased hospitalization time and improoed the quality of life, yet they were nearly equal in wound healing time. The value cannot be estimated as patients may return to work earlier due to early discharge from the hospital, and in children a decrease in anesthesia frequency is due to decreased dressing changes.
