The Saphenous Flap for Coverage of the Below-Knee and the Above-Knee Amputation Stumps
以隱血管皮瓣覆蓋膝下及膝上截肢的殘肢
摘要
下肢截肢手術的目標是儘快復健及早日回到功能性生活。為了達成這個目標，我們設計了隱血管筋膜皮瓣來覆蓋膝下截肢的殘肢，並應用於六位糖尿病併周邊血管阻塞患者身上；此外又以隱血管肌皮瓣覆蓋膝上截肢的殘肢，這是應用於一位糖尿病併膝膕動脈阻塞患者身上。臨床上，隱血管皮瓣傷口癒好良好，疤痕落在接觸面之外，並且殘肢外形平整，可早日套裝義肢及進行復健。如此證明隱血管皮瓣是可靠而穩定的皮瓣。本文並敘述隱血管動脈的解剖位置，並詳敘應用隱血管皮瓣的手術過程。

To achieve the goal of lower extremity amputation surgery for rapid rehabilitation and early return to his functional life, the saphenous fasciocutaneous flap for the coverage of below-knee amputation was designed and applied to six patients, and the saphenous type myocutaneous flap for coverage of the above-knee amputation were designed and applied to one patients. Clinically, good primary wound healing, early prosthesis fitting and walking training were observed. The proper scar location made the whole rehabilitation processes smooth and uneventful. The anatomy of the saphenous artery is described and the surgical technique for the saphenous flap elevation is illustrated.
