Appropriate Impaired Fasting Glucose Level in Taiwan
台灣地區空腹血糖偏高的定義研究
摘要
根據美國糖尿病醫學會定義空腹血糖偏高是指空腹血糖100~125mg/dl。但是就目前的資料而言，並沒有適當的資料來支持這一個定義是否適用於台灣人或是亞洲人。本研究主要是嘗試決定台灣地區空腹血糖偏高的切點。 研究方法：我們收集從2002年1月到2007年7月到大千醫院免費成人健檢至少兩次以上的民眾的健檢資料。本研究排除第一次健檢時就已經有糖尿病或是第一次健檢時空腹血糖≧26 mg/dl的患者。排除這些人後，本研究共有3330個人加入。本研究中新發生糖尿病的定義爲第二次健檢前被診斷爲糖尿病或是第二次健檢時空腹血糖≧126 mg/dl的患者。我們使用接受器操作特性曲線(receiver operating characteristic curve)來計算空腹血糖偏高的切點。 結果：兩次健檢的時間差爲930.68±430.08天。根據接受器操作特性曲線計算的結果，空腹血糖偏高的切點爲90 mg/dl。在接受器操作特性曲線下的面積爲0.777±0.068。 切點處的敏感度和特異度分別爲0.691和0.733。 結論：在台灣空腹敵血糖偏高的切點比美國糖尿病醫學會定義空腹血糖偏高的定義低。我們建議台灣穿腹血糖偏高的切點應該從100 mg/dl減到90 mg/dl而空腹血糖偏高的定義應重新定義爲空腹血糠90~125 mg/dl。

Background: Impaired fasting glucose (IEG) is defined as fasting plasma glucose (FPG) 100 tug dl to 125 mg/dl which was set by American Diabetes Association (ADA). However, there is no suitable data to confirm IEG levels in Taiwan or Asian people. In the study, u e attempt to define suitable cutoff points for IFG in Taiwanese.. Method: We collected data from the people who visited our hospital for health examinations at least twice between 01/2002 and 07/2007. We excluded the people with a history of diabetes mellitus or the people with FPG≧126 mg/dl at first examination. There are 3330 subjects in the study. The criteria for the diagnosis of diabetes in the study are the people with a diagnosis of diabetes mellitus before second examination or FPG≧126 mg dl at second examination. We use the receiver operating characteristic (ROC) curve to calculate the cutoff point in IFG. Results: The mean duration between two health examinations are 930.68±430.08 days. According to the data of the ROC curve, the FPG cutoff point is 90 mg/dl. The area under ROC curve are 0.777±0.068. The sensitivity and specificity for the prediction of diabetes in the cutoff points are 0.691 and 0.733. Conclusion: The cutoff point of IFG is lower in Taiwan than the recommended value set by ADA. We suggest that the cutoff point for IFG should be reduced from 100 mg/dl to 90 mg/dl and that IFG should be redefined as fasting plasma glucose 90 to 125 tug dl in Taiwan.
