上顎竇區鼻竇增高術合併人工植牙治療之骨量變化：四年的追蹤報告
Bone Level Change at Implant-supported Restorations in Conjunction with Sinus Lift Procedures in Maxillary Sinus Area: A 4-year Follow- up Report
摘要
本篇報告探討台北長庚醫院口腔外科，從2000至2004年間在上顎竇區進行鼻竇增高術並植入人工牙根後，四年的長期追蹤結果，以評估在上顎竇區植牙之長期植體成功率及植體周圍骨缺失之變化，並比較性別、年齡、抽菸與否、剩餘齒槽骨多寡及不同鼻竇增高術式與植體周圍骨缺失之變化是否有差異。本研究共有24個病人，包括15位男性及9位女性，平均年齡爲57.83歲，共56支植體。其中有20位患者是接受一階段的骨鑿成形術合併植體同時植入，有4位患者是接受兩階段的鼻竇開窗術，再進行第二階段的植牙。追蹤觀察時間爲四年，分別量測植牙術前及四年後回診之環口X光片或根尖X光片之植體周圍骨缺失，術前患者平均剩餘的齒槽骨高度爲5.67±2.57毫米，平均增進的齒槽高度爲5.07±2.49毫米，人工植體的長度介於10~13毫米，寬度介於3.25~5毫米，平均癒合時間爲一年。四年的追蹤結果發現，共有兩支植體失敗，累積的植體成功率爲96.43%，平均的植體周圍骨缺失在近心側爲0.6±0.31毫米，遠心側爲0.63±0.38毫米；性別、年齡、抽菸與否、剩餘牙嵴高度及不同的術式都不影響植體周圍骨缺失。

The purpose of this retrospective study was to evaluate the peri-implant bone loss of implant-supported restorations in conjunction with sinus lift procedures in maxillary sinus area. Twenty-four posterior partially dentate patients with a total of 54 implants were enrolled in this study, at the department of Oral and Maxillofacial Surgery, Chang-Gung Memorial Hospital, Taipei. From 2000 to 2004, 15 male and 9 female with the mean age was 57.83 y/o. The mean residual ridge height was 5.67±2.57 mm, and the mean augmented ridge height was 5.07±2.49 mm. The implants ranged from 10 to 13 mm in length and from 3.25 to 5 mm in diameter; the healing time for implant was one year. The result revealed mean peri-implant bone loss was 0.6±0.31 mm at mesial aspect, 0.63±0.38 mm at distal aspect, no significant difference was noted despite of age, gender, smoke or not, residual ridge height and different sinus lifting procedures.
