發生在口腔黏膜的黑色素瘤相對於其他身體部位的黑色素瘤所佔的比例極低,大約只有0.7%左右。其中無黑色素黑色素瘤(amelanotic melanoma)又占了12%左右。在文獻報告,其常好發於日本人及烏干達人,而口腔內好發部位爲上顎與上顎牙齦。好發年齡從二十歲到八十歲,男女盛行比率約爲2.8:1.0,遠端轉移之部位則常見於肺部、腦部、肝臟及骨頭。本病例爲一年輕女性因上顎牙齦持續性腫塊而於外院切片檢查證實爲惡性黑色素瘤後轉診至本院門診,因此安排患者接受手術切除,並於術後安排化學免疫療法,且於門診持續追蹤。
Oral mucosal melanoma of the head and neck is a rare tumor that is reported to comprise 0.7% of all other melanomas. Among these, 12% of oral mucosal melanomas are amelanotic. It is reported to occur more frequently in Japan and Uganda. The most frequently affected oral sites are the palate and the upper gingiva. Patients range from 20 to 80 years. Gender ratio is about 2.8:1.0 in male and female. Distant metastasis to the lungs, brain, liver and bones are most frequently observed. In this case, the young female patient came to our OPD due to persistent swelling mass over upper gum and malignant melanoma was proved by incisional biopsy. Then she received surgery followed by immunochemotherapy and keeps on OPD follow up now.
