Aim: A bloodless surgical field is critically important for maintaining precision and avoiding filling material contamination during periapical surgery The purpose of the article is to introduce a novel and simple hemostasis technique for root end preparation and root end filling. Methods: Before root end preparation, dry cottonoid material soaked with 2% lidocaine with 1:80,000 epinephrine is utilized for hemostasis. A case is presented with seven months recall. The simple ”cottonoid technique” perfects root end procedures. Conclusions: There are five advantages in using cottonoid technique in periapical surgery. It provides an alternative hemostasis technique for root end procedures.
