Extra-Articular Diffuse Tenosynovial Giant Cell Tumor of the Temporomandibular Joint-A Case Report
顳顎關節腱鞘滑膜巨細胞瘤－病例報告

腱鞘滑膜巨細胞瘤是相當少見的良性腫瘤，通常發生於四肢關節，可分成侷限性和廣泛性兩種。顳顎關節之腱鞘滑膜巨細胞瘤極為罕見，且通常為廣泛性的關節侵犯破壞。本文描述一位41歲男性，患有顳顎關節外腱鞘滑膜巨細胞瘤，其臨床表徵為一緩慢且局限性之腫瘤，但於病理型態上則呈現廣泛型型態。手術經由廣泛性切除合併部份的關節囊及腮腺切除，術後追蹤一年半無局部復發現象。本文描述此腫瘤的臨床及病理組織型態，以及可做為鑑別診斷之影像檢查及細針穿刺細胞檢查，同時回顧相關文獻。

Tenosynovial giant cell tumors are rare proliferative benign lesions, and are more common in the joints of extremities. Two variants have been distinguished: a localized and a diffuse type. Tumors involving the temporomandibular joint are extremely rare and are often associated with diffuse articular destruction. We present a case of purely extra-articular tenosynovial giant cell tumor of the temporomandibular joint in a 41-year-old man. The clinical patterns and imaging features are indolent which present like a localized type, but it is a diffuse type based on operative findings and histopathologic features. Total excision with partial capsulectomy and parotidectomy was done and no local recurrence was observed during an eighteen-month follow-up period. We characterized the clinical and histopathological features and described the imaging tools and fine-needle aspiration cytology used for the differential diagnosis and operative methods along with a review of the literature.
