Sarcoidosis is a multisystem granulomatous disorder of unknown cause. Subcutaneous sarcoidosis is a rare variant of cutaneous sarcoidosis. Oral corticosteroids are the mainstay of treatment for subcutaneous sarcoidosis. We reported a patient of 69-year-old woman with bilateral subcutaneous sarcoidosis and foreign body granuloma (paraffinoma). Physical examination identified three soft tissue masses in her bilateral cheek. Computed tomography-guided biopsy and incisional biopsy of the skin found granulomatous inflammation, consistent with sarcoidosis and paraffinoma. Using oral steroid treatment and intralesional steroid injection, the lesions improved and were well controlled.
