Objective: To investigate the changes of the anatomical position of the bone Segments by three-dimensional simulation before and after the orthognathic surgery. Methods: In this study, CAD/CAM systems were used to analysis the relationship between preoperative and postoperative bony positions in 15 patients undergoing orthognathic surgery. Results: This study used CAD/CAM system surgical guides to plan the cutting lines. The average time to complete the Le Fort I osteotomy was 48 minutes. The average amount of bone segment deviation was 1.46 mm, and the average amount of deviation on the mesial buccal cusp tip of right upper first molar was 1.76mm. Conclusions: The surgical guide designed in this study can reduce the error resulted from manual measurement. Therefore, by using CAD/CAM system surgical guide technique, we can shorten the operation time of Le Fort I osteotomy and increase the accuracy of positioning of bone fragments.
