When utilizing free flap reconstruction, sometimes the problem of insufficient vascular length either from the recipient site vessels or from the donor site flap pedicle due to repeated surgery or severe trauma may be encountered. Under these circumstances, suitable recipient vessels from a distant region may be found, or a vein graft to establish an arteriovenous shunt loop to overcome the shortage of vascular length can be utilized. Two cases were presented, both of them utilizing the great saphenous vein graft to build the arteriovenous shunt loop which followed by anastomosis with the latissimus dorsi free flap pedicle for reconstruction. Since the operations involved more complicated microvascular reconstruction technique, hence these two cases were presented and the relevant articles reviewed.
