Medial sural artery perforator flap is a thin, pliable and hairless flap which has similar property with radial forearm free flap but has lower donor site morbidity. The flap was first described by Cavadas on 2001 as a refinement of the medial gastrocnemius flap for reconstruction of lower extremities. It is raised from medial part of posterior calf, and the wound can be closed primarily, thus resulting in a better postoperative cosmetic outcome. The flap can be used in different fashions, for example, as local flap or free flap, especially for reconstruction of defect after cancer surgery. This case series reported six cases of oral cancer. After discussing with the patients, we decided to use medial sural artery perforator flap instead of radial forearm free flap for reconstruction. Following the operation of tumor wide excision, the flap was raised for right lower gum, right buccal mucosa, right tongue and right mouth angle defect reconstruction. Postoperative outcome was favorable. Donor site was primary closed and the postoperative cosmetic outcome was satisfying.
