下顎第三大臼齒阻生齒之術後併發症－七年回溯性研究
Complications of Impacted Mandibular Third Molar Extraction-A Seven-year Retrospective Study
摘要
下顎第三大臼齒為所有阻生齒中最為好發之部位；此類患者經常需要手術性拔除患齒。相關於下顎第三大臼齒之常見術後併發症包含術後出血、乾性齒槽炎、手術傷口感染、暫時或永久之舌神經或下齒槽神經損傷。在某些案例中會出現牙齒斷片掉落到組織間隙中，極少數案例報告中會出現下顎骨骨折之併發症。此一研究中統計從西元2016年1月至西元2022年12月於永康奇美醫學中心之下顎第三大臼齒接受齒切手術拔除之術後併發症統計，共七年期間計有8,912部位數中183部位出現併發症，整體併發症發生率為2.05%，並討論其可能之原因、處理方法以及與先前發表之文獻進行比較，以作為未來臨床治療之參考。

Mandibular third molars are one of the most commonly impacted teeth. Surgical extraction of impacted mandibular third molar is a routine procedure of the dental practice. Common postoperative complications associated with mandibular third molar surgical extraction procedure were bleeding, alveolar osteitis, wound infection, temporary or permanent paresthesia of inferior alveolar nerve or lingual nerve. In some occasions, tooth segments dislodged into anatomic space and mandibular fracture rarely reported. This study presents a retrospective review of impacted mandibular third molar between January 1, 2016 and December 31, 2022 in Department of oral and maxillofacial surgery, Chi Mei Medical Center and surveys the incidence of complications. There was a total of 8,912 surgical sites with 183 sites of complications in 7 years period. The overall incidence rate of complication was 2.05%. The possible cause and management of complications were discussed and the result of this study was compared to the previous reports to provide a reference for future treatment.
