運用MTA於開放型根尖成形術─病例報告

Apexification Procedure Using Mineral Trioxide Aggregate-A Case Report

摘要


牙髓受外傷或齲齒甚至感染而發生病變是臨床上接受根管治療的主要原因。而對於牙根尚未完全形成的恆牙,一旦需要接受根管治療時,就要先實施根尖成形術(apexification),待根尖鈣化屏障(apicalbarrier)形成後再進行根管封填。以往根尖成形術是以長期在根管內置放氫氧化鈣(Ca OH 2)來完成,但曠日費時,近來由於生物材料研發的進展,三氧礦聚合物(mineral trioxide aggregate, MTA)因擁有良好的生物相容性和封閉性等特性而被廣泛運用在牙髓病的治療中,所以本病例便以三氧礦聚合物作為恆牙開放根尖的根管封填材料並長期追蹤其治療效果,同時回顧相關文獻。

Apexification was necessary when performed endodontic treatment of open apex teeth. Calcium hydroxide (Ca(OH)2) has been used for apexification for a long time, however, there are some clinical difficulties and risk, such as long treatment time required and the possibility of tooth fracture. Mineral trioxide aggregate (MTA) has good biocompatibility and sealing ability which used widely in endodontic treatment. This case showed successful treatment result in treating a premolar with open apex by using MTA.
