Gummy smile is a common term for excessive gingiva display in which an excessive gingival-to-lip distance of more than 4 mm is classified as unattractive. This condition can be caused by various contributing factors. Despite many efforts to achieve correction in a less-invasive manner, orthognathic surgery is the method with a complex design to use in correcting dentofacial deformities such as maxillary and mandibular prognathisms, abnormal bite relations, temporomandibular joint disorder and receding chins. Combining orthodontic treatment and orthognathic surgery is still the preferred approach to achieve stable occlusion and esthetic outcome with long-term stability, and the design of various surgical techniques is a great challenge to oral and maxillofacial surgeons.
