對於上顎前牙缺牙之病人進行全人治療－植牙周圍炎治療與前牙區植牙缺牙重建：病例報告

摘要
植牙周圍炎（peri-implantitis）歸類在植牙周圍疾病（peri-implant diseases）中，隨著植牙重建缺牙的日益普及，成為臨床常見的疾病。透過有系統的決策圖精準診斷植牙併發症，並擬定相應的治療計劃，可有效的以植牙重建解決缺牙的問題並預防植牙併發症復發。本文以一名上顎前牙缺牙合併複雜缺損骨脊的患者為例，五年前曾經在他家診所接受植牙全口重建療程，唯獨不滿意上顎前牙區以植牙活動式假牙修復，且近期支撐活動假牙的植牙疼痛且有異味，其放射線攝影以及臨床檢查顯示出左上犬齒植體周圍有骨缺損及植體動搖；在移除植體後、垂直骨再生、最終以固定式植牙假牙修復缺齒治療後，進行一年的追蹤檢查。治療部位並無復發的情形，植體周圍健康。最後，本文以相關文獻提及植牙併發症之診斷與治療原則，以及固定式植牙假牙的修復，與此案例做進一步對照與討論。

Peri-implantitis of dental implants are classified as peri-implant diseases. With the increasing popularity of dental implants to reconstruct missing teeth, it has become a common clinical disease. Accurately diagnosing dental implant peri-implantitis through a systematic decision tree and drawing up a corresponding treatment plan can effectively solve the problem of missing teeth with dental implant reconstruction and prevent the recurrence of dental implant peri-implantitis. This article is based on a patient with a missing anterior teeth of the upper jaw and a patient with a complex defect bone ridge. She had received a full-mouth reconstruction treatment at other clinic 5 years ago, but was not satisfied with the removable overdenture at the anterior area of the upper jaw. She also complained of pain and malodor of the implant-supported overdenture recently. Radiographs and clinical examinations showed bony defects around the mobile upper left canine implant and the implant was mobility; after the implant was removed, the bone was reaugmented vertically and finally the missing teeth were replaced with implant-supported restorations. A 1-year follow-up inspection was carried out, there was no recurrence at the treatment site, and the implant was healthy. Finally, this article mentions the diagnosis and treatment principles of dental implant complications and dental implant with related literature, and further contrast and discussion with this case.
