In this case report, two periodontally compromised molars were replaced by dental implants after extraction and ridge preservation. Following soft tissue augmentation, two single implant crowns with customized abutments were then delivered. In two-year clinical and radiographic follow-up, either the surrounding hard and soft tissue or occlusal functions of implants are maintained stable and well-qualified. It is reasonable to conclude that replacing periodontally compromised teeth by dental implants is a predictable treatment option for patients with well-controlled periodontitis, and more cases should be investigated for further confirmation.
