牙科術前使用漱口水對COVID-19病毒的臨床效果探討:文獻回顧

摘要


2019年新型冠狀病毒感染症(Coronavirus Disease 2019, COVID-19)在全球大流行,台灣2021年也開始出現社區傳染,由於口腔黏膜細胞有COVID-19病毒的受體,口腔因而成為疾病傳播途徑之一。牙科治療需要和病患近距離接觸,病毒可能會隨著使用牙科手機、超音波洗牙機和三用噴槍,產生飛沫和氣溶膠傳播,因此在牙科治療前,使用漱口水的觀念因應而至,希望由漱口水的作用降低口中微生物,包括對抗COVID-19病毒,藉此減低牙科病患的病毒傳染能力。目前全世界對於牙科治療前使用漱口水防範COVID-19感染並沒有一致的準則,此篇文章以文獻回顧介紹針對COVID-19常用之漱口水種類,深入探討這些漱口水的作用機轉並評估其臨床效果,以作為牙科治療時,預防COVID-19病毒傳播的參考。

Coronavirus Disease 2019 (COVID-19) has become a global pandemic disease, and Taiwan has entered the community transmission stage. The identification of the COVID-19 virus in saliva and oral mucosa has suggested the oral cavity as a potential reservoir for COVID-19 transmission. Moreover, dental healthcare delivery requires close contact between patients and dental personnel. The dental practice involves the use of rotary dental and surgical instruments, such as handpieces, ultrasonic scalers, and air-water syringes. These procedures create a spray that could contain particle droplets and aerosols of microorganisms. Although pre-dental procedure mouthwashes have been proposed as a strategy to decrease the viral load present in the oral cavity and thus reduce the risk of infectious disease transmission to dental practitioners, there is no consensus guideline for the use of pre-procedure mouth rinses in decreasing COVID-19 transmission clinically. This paper aims to comprehensively review the current use of mouth rinses against COVID-19 infection, and explore their antiseptic mechanisms and clinical efficacy, in order to provide a reference for their potential as a helpful strategy guide in dental procedures.
