Conventional orthognathic surgery treatment requires a prolonged stage of presurgical orthodontic treatment, which usually results in deterioration of facial profile, especially in cases of skeletal Class III occlusion. Recently, the application of surgery-first approach (SFA) minimalizes the stage of presurgical orthodontic treatment and shortens the duration of the whole treatment. The aim of this article is to present a fast-acting treatment of a case with severe class III and anterior open bite in the total treatment duration of 7 months. The 18-year-old girl was characterized with mandibular prognathism, dental Class III malocclusion, slight mandibular spacing, anterior open bite and a lower midline deviation to the right by 1 mm. The treatment was performed with SFA, involving bilateral sagittal split osteotomy (BSSO) surgery, and was followed by orthodontic treatment. The treatment outcome presented an immediate improvement of facial esthetics and good functional occlusion.
