This case report presents a 65 years-old male patient who was diagnosed with chronic periodontitis combined with upper right central incisor cemental tear. The treatments including full mouth subgingival scaling and root planing together with open flap debridement or periodontal regeneration surgery to repair angular bony defects. In terms of oral hygiene instruction and the prevention of illness, we focus on reinforcement of oral hygiene and regular follow-up examination. During one-year follow up, his oral hygiene status has improved greatly. Generally, the gingival tissue had become healthier and all the measurements of probing depthwere less than 5 mm. Radiographic assessment of the angular bony defect of upper right central incisor showed enhancement radiopacity. After cemental tear segment removal combined with periodontal regeneration surgery, the mobility of upper right central incisor was reduced and occlusal function was improved.
