The development of ceramic and adhesive materials has revolutionized many aspects of restorative and aesthetic dentistry. There are many available options to treat peg laterals. This case report presents the aesthetical correction of anterior teeth in a 33-year-old patient who has refused orthodontic treatment. After clinical examinations and the digital smile design planning, we suggested an endodontic treatment completed in the peg-shaped maxillary left lateral incisor. After the anterior guidance alteration, a direct mockup with composite resin was performed afterward to observe the occlusion. During the 4-month follow-up period, we confirmed the stability of the temporary prosthesis. Finally, we established the smile harmony with lithium ceramic laminate and crown to achieve the predictable aesthetic outcome.
