Awareness of dental infection control and risk of disease transmission between dental students and dentists in a teaching hospital

某教學醫院牙醫系學生與牙醫師對牙科感染控制及疾病傳播風險的認知探討

Most clinical educational facilities in dental settings are open-floor plan clinics. After the coronavirus disease (COVID-19) outbreak, the risk in COVID-19 transmission to multiple patients and operators in close proximity is increased and dental infection control operations are extremely important. A cross-sectional study was conducted between dental students and dentists in teaching hospitals in southern Taiwan. This study aimed to explore the differences in knowledge, attitude, and behavior between dental students and dentists following the dental self-assessment policy of dental infection control operations. Further, the risk of disease transmission in an open-floor plan, semi-closed, separated clinic environment, dental chair registration and negative-pressure room from the viewpoints of dental students and dentists was also investigated. A total of 180 dental students and dentists participated in the study. Most participants were male (56.7%), dental students (71.7%) and aged 20-25 years (69.4%). Our investigation showed a high level of knowledge and a careful attitude and behavior regarding the dental self-assessment policy of dental infection control operations among students and dentists. There was no significant difference in the knowledge and attitude of dental students and dentists (P>0.05). The significant difference between the two groups was in the collection of fully documented medical history records (P=0.024). In an open-floor plan situation, all participants believed that there was a high risk of disease transmission. Dental students, compared to dentists, believed that there was a high risk of disease transmission to other patients in separated rooms. There was a significant difference between the two groups (P=0.008). Most participants (>80%) believed that dental chair registration should be required. However, most participants (>60%) did not comment on the negative-pressure room setting. After the coronavirus disease outbreak, dentists and dental students have displayed better compliance rates to infection-control operations and dental chair registration carefully.

醫院牙科臨床教學診間多數為開放式空間。新型冠狀病毒肺炎爆發後,醫病間的近距離接觸容易增加疾病傳播風險,故牙科感染管制更加重要。本研究為橫斷性研究,於南部某教學醫院進行問卷調查,探討牙醫系學生和牙醫師對牙醫院所感染管制作業的知識、態度和行為,對開放、半開放式與獨立空間之牙科診間環境的疾病傳播風險之看法,及對牙科治療椅實名登記與負壓牙科診間之看法。共180位學生與醫師參與本研究,人口學變項以牙醫系學生、男性、年齡20-25歲居多(分別佔全體族群的71.7%、56.7%以及69.4%)。學生和醫師兩組皆具有正確的感染管制知識,態度高度認同與行為遵守度高,兩組在知識與態度上不具有統計學上的顯著差異,但學生會比牙醫師更仔細地詢問病人全身病史並完整登載於病歷中,且具統計學上的顯著性差異(P=0.024)。學生與牙醫師皆高度認同開放式的牙科診間具有高度傳播風險,但學生認為獨立空間對其他病人也具有高度傳播風險,且兩組間有統計學上的顯著性差異(P=0.008)。有8成以上參與者認為應實名登記病人看診之治療椅,但6成以上參與者對負壓牙科診間設置沒有任何意見。本篇結論為疫情爆發後,牙醫系學生與牙醫師均高度遵守感染管制措施且注意到應登記病人看診環境。
