摘要

根管系統易因其形態上的變異導致治療上的遺漏，進而造成根尖周圍炎。上顎第二大臼齒的根管型態，臨床上常見為兩個頰側根管（近心頰側及遠心頰側）與一個顎側根管。當在近心頰側牙根或顎側牙根有額外的根管時，則為臨床上複雜且罕見的根管系統。本文提供兩例上顎第二大臼齒複雜根管系統的病例報告，皆以顯微鏡輔助進行非手術性根管治療並於術後以黏著式陶瓷復型（bonded porcelain restoration）完成治療。

Missed canal during endodontic treatment on account of different root canal configuration will cause subsequent apical periodontitis. Canal morphology in maxillary second molar typically was known as two buccal canals (mesio-buccal and disto-buccal) and one palatal canal. Additional canals in mesial or palatal root were clinically recognized as a difficult and rare case. This literature provides two cases reports for complicated canal configuration in maxillary second molar receiving non-surgical root canal treatment under microscope-assisted and restored with bonded porcelain restoration.
