露齦笑之跨科處理方法及考量：臨床病例報告
Case report: Interdisciplinary management and consideration of Gummy smile
摘要
露齦笑是指牙齦過度露出，有時會讓患者感到不舒服，而造成露齦笑的原因有下顎發育問題、牙齒萌出異常、咬合不當、上唇過度活躍、肌肉問題等，需要找出根本的原因才能解決它。病患來診主訴為覺得自己的牙齒很短很暴且難看，齒間縫隙明顯，因此想藉由重新製作假牙的方式來解決長期以來的困擾。患者為骨性及安格式二級異常咬合，凸臉型、上顎骨過度生長以及下顎骨生長不足，嘴唇不易閉合且有露齦笑。透過階段治療，包括矯正、正顎手術、牙冠增長術和贋復物重新製作，治療完成後，病人的雙唇可以輕鬆自然地閉合，開心的微笑，治療結果滿足病人對美學的期望，跨專科的協同評估及治療，對於改善多因素造成露齦笑的美觀效果是必要的。

Gummy smile means excessive exposure of gums, which sometimes makes patients feel uncomfortable. The causes of gummy smile include mandibular development problems, abnormal tooth eruption, improper bite, overactive upper lip, muscle problems, etc. The origin cause needs to be found. The patient comes to the clinic complaining that her teeth are short, buck and ugly, with obvious gaps between the teeth. Therefore, she wants to solve the long-term problem by making new prosthesis. The patient was skeletal and Angular class II malocclusion, a convex face, overgrowth of the maxillary bone, and undergrowth of the mandibular bone. The lips were difficult to close and had a gummy smile. Through staged treatment, including correction, orthognathic surgery, crown lengthening and prostheis reconstruction, the patient's lips can close easily and naturally, with a happy smile after the treatment is completed. The treatment results meet the patient's aesthetic expectations. Interdisciplinary collaborative evaluation and treatment are necessary to improve the aesthetics of gummy smiles caused by multiple factors.
