後牙近心傾斜合併牙周病的矯正治療－病例報告
Orthodontic Treatment on Molar Mesial Tipping with Periodontitis - A Case Report
摘要
矯正治療的目的是讓牙齒適當地移動，達到顏面部和牙齒排列上的美觀以及改善咬合功能。但隨著成人矯正患者人數成長，矯正醫師也面臨較多的牙周病的患者。本報告為後牙近心傾斜合併牙周病的矯正治療成功案例，病人為47歲女性，主訴為想要贋復假牙，經檢查發現病人下頷雙側第一大臼齒缺失引致第二，第三大臼齒近心傾斜達60度。SNA為87度，Wits值為-13mm，ANB為3度，犬齒呈二級異常咬合，前牙垂直覆咬正常，水平覆咬正常，診斷為骨性二級異常咬合。經第一階段牙周病治療，並在定期清創以及追蹤的情況下，進行矯正治療，矯正期間每三個月定期牙周回診，經過48個月的矯正治療，牙周狀況維持穩定，牙齒扶正，經植牙後咬合功能得到改善。適當的矯正治療配合良好的牙周治療以及回診，能夠得到具功能與美觀的咬合。

Orthodontic treatment aims to realign teeth to achieve both aesthetic and functional improvements in facial and dental alignment. With the growing number of adult patients seeking orthodontic care, many present with periodontal challenges that require careful management. This report highlights a successful case of orthodontic treatment for a 47-year-old female patient with mesial inclination of posterior teeth and periodontal disease. The patient sought to restore missing teeth, and examination revealed bilateral mandibular first molar loss, causing a 60-degree mesial inclination of the second and third molars. Diagnostic findings included an SNA of 87 degrees, a Wit's value of -13 mm, and an ANB of 3 degrees, consistent with skeletal Class II malocclusion with normal anterior overbite and overjet. Orthodontic treatment began following initial periodontal therapy, complemented by regular scaling and monitoring. Periodontal evaluations were conducted every three months during the 48-month treatment period, ensuring stable periodontal health. The outcome was successful alignment of the teeth, improved occlusion, and the placement of dental implants. This case underscores that combining orthodontic and periodontal therapies, with diligent follow-up, can effectively achieve both functional and aesthetic occlusal outcomes.
