Postauricular Dermoid Cyst and Reconstruction of Wound Using the Fascio-Cutaneous Flap - A Case Report and Literature Review

摘要

Background: Dermoid cyst is a benign cystic lesion uncommonly found in the head and neck region. Manifestation of patients vary, normally there are no symptoms, but patients may feel pain if there is tissue inflammation or local infection. Treatment of dermoid cyst is complete surgical excision to prevent recurrence. Herein, we report a case of a male adult with a dermoid cyst at left postauricular area. The clinical presentation of the patient, diagnosis and treatment with follow up were detailly included. Aim and Objectives: We reported a case of a 33-year-old male who complained of left postauricular mass for years. The mass was non-tendered and there was no sign of acute inflammation or infection. A final diagnosis of post auricular dermoid cyst was made. This case report was made for clinicians to be more careful in establishing the diagnosis so that patients can receive the right therapy. Materials and Methods: Surgical intervention with complete surgical excision of left postauricular mass was performed. Wound is reconstructed with local fascio-cutaneous flap. Results: The specimen was sent for pathological check. Histopathological examination confirmed the diagnosis of a dermoid cyst. No perioperative complications were noted. The patient was discharged and under close surveillance at the out-patient department (OPD) with good aesthetic outcome. There is no evidence of recurrence. Conclusion: Although postauricular dermoid cyst is rare, it should be a potential consideration on the list of differential diagnosis. Surgical intervention with complete excision provides a good outcome with low recurrence rate. Moreover, wound reconstruction with local fascio-cutaneous flap provides good wound healing with satisfied aesthetic outcome.

背景：皮樣囊腫是一種良性囊性病變，很少見於頭頸部區域。患者的表現各不相同，一般沒有症狀，但如果有組織發炎或局部感染，患者可能會感到疼痛。皮樣囊腫的治療需要徹底手術切除以防止複發。在此，我們報告一例患有左側耳後皮樣囊腫的成年男性。並詳細介紹患者的臨床表現、診斷和治療。目的及目標：我們描述一名33歲的男性，主訴為左側耳後腫塊持續多年。腫塊沒有壓痛，沒有急性發炎症狀或感染的跡象。患者接受手術後症狀改善，預後良好。材料及方法：這名33歲男性接受左側耳後腫塊切除手術並使用局部筋膜皮瓣重建傷口。檢體被送去病理化驗。組織病理學檢查證實腫塊為皮樣囊腫。結果：術後並未發現任何併發症。患者出院並在門診部接受密切監測，沒有發現腫瘤復發的證據。結論：耳後皮樣囊腫非常罕見，但當患者以耳後腫瘤來求醫時，耳後皮樣囊腫應被考慮為鑑別診斷之一。完全囊腫切除手術才能提供良好的預後及低復發率。此外，利用局部筋膜皮瓣重建傷口可以提供良好的傷口癒合及令人滿意的術後外觀。
