摘要


不對稱的哭相是近十餘年來漸受重視的先天性面部表情異常,其特徵是當患者面部無表時看似正常,但是當患者啼哭或展露笑容時,嘴角偏針向一側而形成此特殊的面部表情。引起嘴角偏針的原因,可能是單側的降口角肌先天性缺損或發育不全。本症常合併各種先天性畸形。 本文報告2例不對稱哭相之病例。此2例都具有典型的不對稱哭相的面部表情。其中1例並有肌電圖佐證。除了有不對稱的哭相之外,此二病例均有先天性心臓病以及其他先天性畸形。著者等將就有關文獻提出討論。

Asymmetric crying facies is a recently recognized congenital facial abnormality. It is frequently associated with congenital cardiovascular and other anomalies. The diagnosis is mainly based upon clinical features and electromyography (EMG). The most important differential diagnosis is congenital facial palsy due to birth trauma and maldevelopment of the seventh cranial nerve. The disorder is thought to be due to hypoplasia of the depressor angularis oris muscle (DAOM). Two typical cases of asymmetric crying facies are reported here. Both had congenital heart disease and other congenital anomalies. The first case had a parasitic twin. EMG study in one case confirmed dysfunction of the unilateral DAOM. The pertinent literature is reviewed.
