小兒中樞神經性疾患之腦脊髓液內乳酸脫氫酶活性及其異酶之研究

摘要


爲了要了解小兒中樞神經性疾患時,其腦脊髓液中的乳酸脫氫酶活性(以下簡稱LDH)及其異酶的變化,本實驗共收集128個檢體,測定其LDH活性及異酶分佈。其結果如下:1.對照組的LDH活性及爲14.9±8.0IU/L,異酶型熊以LDH1、LDH2爲主。2.非感染性中樞神經疾患中,熱性痙攣者的LDH活性及異酶分佈與對照組相似,而痙攣及腦症病人的LDH活性分別爲17.4±9.0IU/L及24.9±18.0IU/L,異酶分佈與對照組一致。3.所有感染性中樞神經疾患的病例(包括無菌性、化膿性、嗜伊紅性及結核性腦膜炎),其LDH活性均上昇,其中以化膿性腦膜炎者最高237±252.8IU/L,異酶的分佈型態:化膿性腦膜炎以LDH4、LDH5最明顯,無菌性腦膜炎以LDH2、LDH3爲主,結核性腦膜炎LDH2、LDH3較高、而嗜伊紅性腦膜炎則五個異酶平均分佈。

One hundred and twenty-eight cerebrospinal fluid (CSF) specimens were analyzed for total lactate dehydrogenase (LDH) activity and its isoenzyme distribution. The LDH activities were elevated in the CSF of the patients with meningitis and encephalopathy, especially in bacterial meningitis, The isoenzymes of the CSF in patients with bacterial meningitis were predominant in the fractions 4 and 5, which are most likely released from granulocytes in the CSF While in patients with aseptic meningitis, with predominence of lymphocytes in the CSF, the isoenzymes 1, 2 and 3 were elevated In eosinophilic meningitis, the 1 DH activities ware increased with elevation of the isoenzymes 4 and 5. In tuberculous meningitis, the isoenzyme patterns showed elevation of LDH 2 and LDH 3 with predominance of the lymphocytes in the CSF. The LDH isoenzymes 1, 2 and 3 were increaser in the cases of encephalopathy, and their LDH activities were higher than those of the controls, but lower than those of meningitis. Mild increase of the CSF LDII activity also occurred in the patients with seizures. However, no significant changes of the CSF LDH activity and its isoenzymes were noted in the children with febrile convulsions. There was no correlation between CSF LDH activity and its total cell count, protein and glucose.
