Visualization of Patent Ductus Arteriosus by Subxyphoid Two-Dimensional Echocardiography

摘要


利用超音波心圖儀從左胸前方來直接偵測存開性動脈導管的存在仍然是不太容易。本研究乃使用Toshiba Sonolayer-V model SSL-53M 的雙向弧型超音波檢查儀及2.4MHz可變焦距的掃描器,對13名5歲以下懷疑患有此症的兒童及30名患有心室中膈缺損的兒童(對照羣)施行劍突下方的心臟超音彼檢查。此法可以窺見存開性動脈導管者有12名(92.3%)其直徑為0.3公分至1.4公分。此13名患童全部經由心導管術證實動脈導管的存在,其中並有10名患童接受手術治療。施行血管攝影衛發現動脈導管的直徑與超音波檢查之直徑相差不多,γ值為0.59。本研究證明劍突下方雙向孤型超音波檢查可以正確的直接偵測動脈導管的存在並能測定其直徑的大小。

Direct visualization of the patent ductus arteriosus (PDA) by two-dimensional echocardiography (2DE) is still a challenging problem in children. In this study, 13 patients (aged from 16 days to 8 yrs 10 mos) suspected of having PDA (group I) were examined, as were 30 patients (aged from 2 mos to 10 yrs 8 mos) suspected of having VSD as a control group (group Ⅱ); an electronic sector scanner by subxyphoid approach was used. After the standard subxyphoid 4-chamber view was obtained, the transducer was tilted cranially toward the left ventricular outflow tract and ascending aorta. In this view, the junction of both right and left pulmonary arteries and ductus arteriosus could be visualized when present. All but one in group I (92.3%) were correctly diagnosed with this technique by direct visualization of the ductus. All patients were catheterized and 10 had surgical intervention. The diameter of the ductus arteriosus measured by subxyphoid 2DE correlated with that of angiographic findings with a correlation coefficient of 0.59. Postoperatively, an unusual echo density was visualized at the previous ductal area. This dense echo was thought to be due to suture materials and local tissue reaction.
