學童之尿鈣排泄與高尿鈣篩檢的研究

A Study on the Urinary Calcium Excretion and Screeing for Hypercalciuria in School Children

本研究在對6~18歲間之正常學童於未進食早餐之空腹時(1,049人),上午8時至下午4時之8小時(1,074人)及上午8時至次日上午8時之24小時(127人)的尿液中鈣、肌酸酐等生化物的分析中發現;尿鈣與肌酸酐的比值(Ca/Cr)平均分別為0.070±0.045, 0.093±0.055及0.096±0.0550。尿鈣在24小時的排泄量平均為2.06±1.15mg/kg。若以mean±2SD為其上限的標凖時,則在空腹,8小時及24小時尿液中Ca/Cr值分別為0.16 0.21及0.21,而24小時尿鈣排泄量之上限則為4.4mg/kg。若以超過此上限值(卽尿液中Ca/Cr值大於了0.21,或尿鈣排泄量大於了4.4mg/kg/day)做為高尿鈣節檢的標準時,則本研究之學童高尿鈣發生率約為4.2%,其中仍以空腹時之尿液中Ca/Cr值為高尿鈣篩檢的較佳指標。若篩檢中疑為高尿鈣者時,則需以24小時尿鈣排泄量做更進一步的證實。

For the purpose of understanding of the incidence of hypercalciuria in Chinese school children, a quantitative study of urinary calcium and creatinine on fasting morning urine (1049 cases), 8-hour urine (1074 cases) and 24-hou,r urine (127 cases) from 6 to 18-year-old school children was performed. The results showed that the mean urinary calcium/creatinine ratios (Ca/Cr) of fasting morning urine, 8-hour urine and 24-hour urine were 0.070±0.045, 0.093±0.05, 0.096±0.055, respectively. The mean 24-hour urinary calcium excretion of 127 children was 2.06±1.15mg/kg. If mean+2SD is taken to represent the normal upper limit, 0.21 is the normal upper limit for Ca/Cr, and 4.4mg/kg for 24-hour calcium excretion. When taking over 0.21 for Ca/Cr or over 4.4 mg/kg/day as the index of hypercalciuria, the incidence of hypercalciuria in our children is 4.2%. We suggest that the 24-hour urinary calcium excretion is the best parameter of hypercalciuria and the ratio of urinary calcium and creatinine (Ca/Cr) on fasting morning urine is a good screening test for hypercalciuria.
