Clinical Observation of Post-Asphyxial Syndrome in Full Term Neonates

足產兒窒息後症候羣之臨床觀察

摘要


自民國73年1月至75年1月,共有37位足產兒因窒息而住院於彰化基督教醫院之新生兒加護病房。其中25位是男嬰,12位是女嬰。造成這些嬰兒窒息的危險因素計有:羊水中有胎便(35.1%);原因不明(29.7%);臀位或胎位不正常(13.5%)等等。主要之後遺症及臨床症狀如次:(1)中樞神經系統:無力(72.9);Moro's反射消失或減低(56.7%);吸食力弱(54%);體溫不穩(45.9%);呼吸暫停(43.2%);抽搐(27%);腦浮腫(27%);腦出血(13.6%)。(2)呼吸系統:呼吸衰竭(45.9%);胎便吸入症(35.1%);氣胸(8.1%);持續性高肺動脈壓症(8.1%);肺出血(2.7%)。(3)心臟血管系統:低血壓(27%);心臟衰竭(2.7%)。(4)腎衰竭:16.2%。(5)電解質及代謝異常:低血鈣(19.4%);低血鈉(11.4%);高血鉀(0%);酸血病(29.7%);低血糖(8.3%)。(6)血凝固異常:3位病人呈PT及APTT之延長現象。(7)高膽紅素症:29.7%。這些病兒之死亡率為21.6%(8/37)。約17.2%(5/29)的存活病例呈明顯之神經傷害。而31%(9/29)發現有腦波異常的現象。

Between January 1984 and January 1986, a total of 37 full term neonates suffering from asphyxia at birth were admitted to the neonatal unit of Changhua Christian Hospital. Twenty-five were male and 12 female. The main predisposing perinatal risk factors were meconium staining of the amniotic fluid (35.1%), breech and abnormal presentation (13.5%), and unknown cause (29.7%). Salient manifestations and complications of different systems included: (1) central nervous system (CIVS) complications: hypotonia (72.9%), incomplete or absent Moro's reflex (56.7%), weak sucking (54%), temperature fluctuation (45.9%), apnea (43.2%), seizure (27%), brain swelling (27%), hemorrhage (13.6%); (2) Pulmonary complications: respiratory failure (45.9%), meconium aspiration syndrome (35.1), pneumothorax (8.1%), persistent pulmonary hemorrhage (2.7%); (3) cardiovascular complications: hyftotension (27%), congestive heart failure (2.7%); (4) Intrinsic renal failure (16.2%); (5) Electrolyte and metalic disorders: hypocalcemia (19.4%), hyponatremia (11.4%), hyperkalemia (0%), L;idosis (29.7%), hypoglycemia (8.3%); (6) Abnormal coagulation: unknown (89.2%), 3 cases had prolonged prothrombin time (PT) & activated partial thromboplastin time (APTT); (7) Hyperbilirubinemia (29.7%). The mortality rate was 21.6% (8/37). 17.2% (5/29) of the survivals showed obvious neurological sequelae and 31% (9/29) revealed EEG abnormalities.
