海洋性貧血患童ABO血型不合之異體骨髓移植

摘要


為了研究ABO血型不合在異體骨髓移植所扮演的角色。我們收集省立桃園醫院向民國73年3月至74年12月,一年九個月間,所做11例重型海洋性貧血患童的異體骨髓移植,有5例屬於ABO血型不合的病例。骨髓與血液混合液利用冷凍離心機,血漿萃取器,除去血漿,再將供髓者的濃積性紅血球和血沈棕黃層(buffy coat)輸給受髓者。其中有3例產生自源性恢復(autogenous recovery),1例因植體對抗宿主疾病(Graft-versus-host disease)加上全身性巨細胞病毒(cytomegalovirus)感染而死亡。成功的病例為一8歲大的男孩,經血紅素電泳法和血型證實完全痊癒。現已追綜一年以上。我們比較ABO血型適合與不適合的移植病例,其發生植體對抗宿主疾病的發生率,長著時間(engraftment time),血液成份輸血的次數;發現兩者並沒有統計學上的差異。特參考文獻,提出報告。

Among 11 patients who underwent allogeneic bone marrow transplantation (BMT) at Tao-Yuan Provincial Hospital from March 1984 to December 1985, five cases received marrow from ABO incompatible donors. Removal of plasma was done before transfusing the marrow cells to recipients via right atrial catheter. Three cases resulted in autogenous recovery, one case died of grade IV graft versus host disease (GVHD) with systemic cytomegalovirus infection on day 55. An 8-year-old boy has sustained engraftment which was confirmed by hemoglobin electrophoresis and the change of blood type. Comparison of the incidence of GVHD, engraftment time, blood product requirement between ABO-compatible and incompatible transplants revealed no significant difference.
