由歷史瞻望台灣地區新生兒加護醫療之需求

The Need for Neonatal Intensive Care in Taiwan: Historical Perspectives

摘要


近年生命統計資料顯示,由於新生兒加護醫療的時步,世界各國的嬰兒死亡率都呈明顯下降的趨勢,目前我國在內的一些較進步的開發中國家,已經積極地發展新生兒加護醫療。然而,由於發展經費昂貴,所需工作人員衆多,且能否比加強兒童期傳染病管制有更好的投資效益等原因,使得新生兒加護醫療有無必要優先推展受到質疑。本文藉由西方先進國家新生兒加護醫療演進的歷史過程,參照我國的現況和統計資料,闡明我國新生兒加護醫療可能發展的方向,並概估其人力和經費的需求,以供爾後研究及當局施政的參考。

Recent vital statistics data reveals that in most of the developed countries the infant mortality has decreased reaching less than 15 per 1000 live births. The major component of this decline has been in neonatal mortality, which has been brought by neonatal intensive care. Presently a steady movement for the development of neonatal intensive care in some of the leading developing countries, including Taiwan area of Republic of China has been noticed. In these countries, however, a question has risen as to priority and justification for developing neonatal intensive care. This stems mostly from relatively high cost and additional health care manpower required for the neonatal intensive care. This communication reviews the evolution of neonatal intensive care in the Western World and with this historic perspective, attempts to assess the need of neonatal intensive care in Taiwan area. The results also could be used as a basis for future rsearch and public health policy.
