減肥菜導致阻塞性肺疾病,除帶給病患身體痛苦外也伴隨優傷、無價值、自責……等一連串心理反應,導致無力感産生。本文即描述一位因減肥菜中毒而産生阻塞性肺疾病合併呼吸衰竭的45歲婦女在面對所有治療皆無對效時,對自己進食減肥菜導致家庭結構及經濟改變感到自責與內疚,對疾病及未來感到茫然、無力感的個案。筆者籍由理論與護理過程的運用,以體力及身體抵抗力、精力及社會支持、正向自我概念、能量、知識、動機及信念爲中心概念,並攷慮個案的特異性情境因素,協助個案正視本身的無力感,提供適當的自我控制,進而發展希望、培養希望,克服情緒失落及生活障礙,使病患能再重新尋求人生意義及自我價值。本報告可提供臨床護理人員在面對及處理病患無力感時的參攷。
Weight reducing vegetables may induce obstructive lung disease. In addition to physical Suffering, these diseases are accompanied by depression, feelings of worthlessness, self-blame, and psychological imbalance. All these symptoms may result in a sense of powerlessness. This article describes a 45 year-old woman who had eaten poisonous Sauropus androgynus vegetables which induced obstructive lung disease complicated by respiratory failure. Because her condition was irreversible after medical treatment, she experienced self-blame and guilt about the change of her family structure and economy resulting from eating Sauropus androgynus. She felt at a loss and powerless regarding her illness and future. The researcher used the powerlessness theory and the nursing process as a central conception coupled with physical strength, psychological stamina and social support, positive self-concept, energy, knowledge, motivation, and belief system to help the patient. Specific and situational factors were considered to help her to face up to her sense of powerlessness. The researcher also provided appropriate self-control to develop and foster her hope, in order to cope with the sense of loss and life difficulties. By this, the researcher hoped the patient could refind her life meanings and self-worth. The study can thus provide a reference for nurses taking care of patients with feelings of powerlessness.
