摘要

本文敘述一位急性心肌梗塞個案執行經皮冠狀動脈介入治療後，筆者運用護理措施順利緩解其背痛之照護經驗，照護期間為2008年9月5日到9月7日。個案在經皮冠狀動脈介入治療後因長時間平躺限制活動，引起一連串背部及肢體的不適、睡眠干擾、及焦慮的情緒反應。筆者運用翻身擺位、按摩及床上肢體運動等措施，並指導家屬共同參與照護，明顯的改善背痛，提升夜眠品質、進而緩解其焦慮。藉由本個案照護之經驗，本文提出針對平躺限制活動患者防範及緩解背痛之照護建議。

The article reports on the experience of the author in successfully using nursing interventions to relieve the back pain of a patient who received percutaneous coronary intervention (PCI) for acute myocardial infarction. Nursing interventions were administered over three days, from September 5~7, 2008. After PCI, the patient suffered from back pain, insomnia and anxiety due to prolonged immobility and restricted positioning. The author used nursing interventions, such as position changing, massage and in situ limb movements to relieve symptoms, and instructed family members to participate in caregiving. Back pain and quality of sleep significantly improved, which greatly relieved the patient's anxiety and improved sleep quality. This article suggests specific nursing intervention techniques to relieve back pain induced by prolonged immobility/restricted positioning.
