台灣中部民衆就醫及用藥概況調查－以某醫學中心門診病患為例

Survey on Adult Behavior and Medication of Chinese and Western Medicine-Based on the Outpatients of a Medical Center in Mid-Taiwan

摘要

民眾的就醫及用藥概況、中西藥物使用習慣、中西藥物併用及導致不良反應等情況急待瞭解，因此本研究針對台灣中部某醫學中心門診病患進行問卷調查，期能瞭解台灣中部民眾就醫、用藥習慣與認知及中西藥物併用情形。本研究以中部某醫學中心十八歲以上之門診病患爲隨機抽樣對象，透過一對一方式進行訪談，總計回收問卷2176份，其中中醫門診收集1132份，西醫門診收集1044份，其中91.7%爲有效問卷計1995份。
研究結果發現：醫院的醫療品質及距離是民眾就醫的主要考量因素。就醫過程中曾更換醫師者，以女性、大專以上教育程度、未婚、40歲以下及有職業之受訪者較多。慢性支氣管炎、消化性潰瘍及過敏性疾病等病患，更換醫師之比例較高。六成以上的受訪者表示對藥物副作用大部分不了解，高血壓、胃腸功能障礙及慢性肝炎、肝硬化病患是不了解藥效及副作用的高危險群。藥效及副作用之資訊來源主要來自醫師。受訪者中過去三年只看西醫的佔37.3%，只看西醫的佔9.8%，曾看中醫與西醫的佔52.8%。在兼看中醫與西醫的受訪者中，三年內曾同一天服用中藥與西藥者佔61.5%。台灣中部民眾同期併服中西藥的高發生率不容忽視，中西藥物併用對療效與安全的影響值得研究。

This study was to investigate the adult habits of utilizing Chinese and western medical clinics and the prevalence of concurrent use of Chinese and western medicine. Structural questionnaires were used to survey the outpatients with age over 18 of a medical center in middle Taiwan. A total of 2176 questionnaires were collected, including 1132 and 1044 from Chinese and western medical clinics, respectively, among them 91.7% were effective. The results indicate that medical care quality and distance of hospitals from home are the major factors for patients to choose their hospitals. Unmarried, female, under 40 or higher educated people are prone to change medical doctors for their caring. The knowledge of drug safety and side effects of patients was chiefly obtained from medical doctors. During the past 3 years, 37.3% and 9.8% of the surveyed outpatients visited only western and Chinese medical clinics, respectively, and 52.8% visited both types of clinics. Among the last group, 61.5% have taken Chinese and western medicines at the same day. The high prevalence of concurrent use of Chinese and western medicine should not be overlooked. The herb-drug interactions warrant more studies for drug efficacy and safety.
