傾食症候群中西結合治療病例

本病例是一位傾食症候群的51歲婦女，曾罹患重度憂鬱症，因喝鹽酸(HCl)致食道及胃損傷，先後兩次部分胃切除及食道換大腸手術，後由於胃與食道重建手術之縫合傷口處發生狹窄及胃酸逆流，復行胃繞道手術，使食物直接由食道進入小腸，此前後三次開刀手術，引起脾胃內傷，中焦氣陷，氣機紊亂，而發生傾食症候群（Downping syndrom）。由於食後易引起低血糖、虛脫、甚至暈厥，西醫曾給與Acarbose 治療，以延遲並減少飯後血糖升高，但患者感覺療效不彰，因而轉而至中醫門診，以補中益氣湯飲片為主，出入加減治療，患者並自己改變進食方法及口服中藥的時間，經調治一年半左右，結果傾食症候群完全緩解。追蹤三年未再復發。

The present case was a fifty one year's woman of dumping syndrome. She was suffered advanced depression and drank hydrochronic acid caused esophageal and gastric damage. So she received partial gastrectomy, esophageal reconstruction by BII anastomosis, rouex -Y and pedicled colon, as well as redon roux Y bypass was redo and the inlet was connected to the distal part of the pedicled colon segment and jejunostomy. The three times of operative result influenced that the spleen-stomack internal damage, qi falling of the middle burner, disturbance of qi mechanism and so caused dumping syndrome. The patient was easily induced hypoglycemia, weakness and syncope after oral feeding. So western's doctor gave an acarbose treatment to delay and decreased postprandial blood sugar level, but had not been apparent effect. She was transferred to the Chinese department and received Chinese Medicine treatment with center-supplementing qi boosting decoction pieces. She also changed intake food method and regulatory oral feeding herbs time. The result shown dumping syndrome was completed remission after a year and half therapy. Track three years without recurrence.
