他覺性搏動耳鳴：病例報告
Objective Pulsatile Tinnitus: A Case Report
摘要
他覺性耳鳴起因於血管先天異常或病變者引起和脈搏搏動一致的耳鳴，它的特徵除了患者本人能聽到耳鳴聲外，檢查者亦可同時聽到。本院耳鼻喉科經歷一此類型病例，有一已婚女性，於第二胎懷孕中開始有搏動性耳鳴的症狀出現，生產後症狀仍然持續在且加重。作腦血管攝影檢查，發現在中腦膜動脈與海綿竇間及內項動脈與海綿鑟間各有一條瘻簵相通。第一次手術為她作外頸動脈結紮，耳鳴瞬即消失，但隔日又發，因此於第一次手術後五天復作內頸脈結紮，耳鳴症狀迅即獲得改善。於術後半年、九個月及一年的追踪檢查，耳鳴症狀完全消失，且無復發性形。

Objective tinnitus is rate. Unlike subjective tinnitus, it may be perceived not only by the patient bu also by the examiner on auscultation of the auditory canal or the neighboring structures. Objective tinnitus, which is caused by a vascular abnormality, is usually palsatile. The possibility of a vascular abnormality can be confirmed by the use of angiography. We present a 23-year-old female with left objective pulsatile tinnitus, which occurred during her second pregnancy. It persisted, aggravated, and even interfered with her and her husband’s sleep. Carotid angiography revealed 2 fistulas. One was between the left middle meningeal artery and the left cavernous sinus, and the other was between the left internal carotid artery and the left cavernous sinus. The tinnitus was much improved after the ligation of the left external carotid artery and the left internal jugular vein. There was no recurrence in the following year.
