局部界限性淋巴管瘤：病例報告
Localized Lymphangioma Circumscriptum
摘要
患者為為壹位33歲男性，於其左上臂伸側有一約0.9×0.7公分的淡褐色彈性結節，約有貳個月病史；且病灶有逐漸擴大的現象。病灶經切除後，其病理組織顯示在真皮的上層有多數管腔狀之構造，這些管腔由一層內皮樣細胞所圍繞，細胞之間隔較遠，而其外圍沒有周皮細胞之存在，顯示此腫瘤為一淋巴管之增生。而上述病理組織之Factor VIII R Ag的PAP染色顯示管腔內皮呈現陰性反應，而其周圍之正常微血管則呈現強陽性反應，因此診斷此病為一局部界限性淋巴管瘤。由於此病例的臨床表現與典型之淋巴管瘤顯著不同，乃提出報告。

Lymphangioma circumscriptum is a rare disorder that is localized on the skin. Clinically, it often appears in groups of vesicles containing clear or bloody fluid. This report describes a case of localized lymphangioma ccircumscriptum. The case appeared as a 0.9×0.7 cm elastic brownish nodule. Multiple vascular spaces were noted in the upper dermis under histopathological examination. Factor VIIIR Ag was not found in the luminar structures by PAP staining method. The reporting of such a case presents a clear clinical picture of this rare disorder. It is hoped that this report will encourage an increased awareness of the occurrence of lymphangioma circumscriptum.
