Amiodarone治療心室性早期收縮之臨床經驗

Clinical Experiences in the Use of Amiodarone in the Suppression of Ventricular Premature Contractions

Between February 1983 and February 1985, a total of 26 cases of symptomatic ventricular arrhythmias were treated with amiodarone at the Department of Internal Medicine, Kaohsiung Medical College in order to investigate the clinical pharmacology of this drug. Amiodarone was used with oral/or intravenous route. The oral administration was studied in 23 cases (including cases of combined use), the therapeutic efficacy was 78% (18/23) with a mean follow-up period of 9.2 weeks. The intravenous administration of amiodarone was studied in 7 cases, the therapeutic efficacy was 57% (4/7). In 13 of the 26 cases TSH significantly increased from 2.59 ± 0.85 to 3.83μu (P<0.005), T4 from 7.39 ± 71.85 to 8.62 ±1.45μu (P<0.05) and FTI from 2.67±0.37 to 3.15 ± 0.57(p<0.005)after an average of 47 days of amiodarone therapy. Two cases were noted to develop corneal microdeposits. Sixteen of 18 cases with therapeutic efficacy had 90% or more reduction in the frequency of ventricular premature contractions (VPCs) within one week following oral administration of amiodarone. The duration for a 90% or more reduction of VPC frequency after intravenous injection of amiodarone raged from 8 minutes to 17 hours with a mean of 9.78 ± 8.12 hours. Its efficacy and long half-life make it an agent of practial value in suppressing ventricular arrhythmias.
