Diagnostic Value of Sural Nerve Biopsy and Electrophysiological Studies in n-Hexane Intoxication

本文報告一位16歲男性病人，一年來罹患二次四肢肌肉無力和指、趾末端麻木。臨床神經檢查發現中度肌肉無力，指間肌肉萎縮、套襪型感覺障礙及全身肌腱反射消失。電器生理檢察呈現運動神經傳導速度顯著變慢、肌肉誘發電位降低和遠端潛時延長。臨床上被診斷為慢性復發之末梢神經炎。然而腓腸神經切片檢查發現典型的中毒性末梢神經病變，如腫大之神經軸突和分斷性脫髓鞘病變。此種混合軸突和髓鞘之病理神經切片檢查對此類原因不明之末梢神經病變患者診斷的重要性。

As utilization of radioimmunoassay becomes increasingly popular, so too the amount of laboratory tasks and documenting tasks. In this study an ABBOTT ANSR gammacounter was connected with a TATUNG TCS-5000 microcomputer to record the output cpm and to calculate the cut-off value. The result of assays were on-line stored on the diskettes, and than transferred to the main database. By this on –line system the loading of document key-in task was decreased, enhancing the accuracy of transfer of the results and decreasing the possibility of human error. Developing a database management if another joy of this laboratory, and the details of such a job will be described in the near future. A 16 year-old male developed recurrent muscle weakness and distal numbness of all 4 limbs for 1 year. A moderate degree of muscle weakness with intrinsic hand muscle atrophy, glove and stocking type sensory disturbance and generalized areflexia were found. Electrophysiological studies showed marked showing of motor never conduction velocities (mNCV), reduction of amplitudes in muscle action potentials (MAP) and prolongation of distal latencies (DL). He was diagnosed to have a chronic relapsing inflammatory polyneuropathy. However the biopsied sural nerve specimen revealed a characteristic finding of giant axon with axonal degeneration and segmental demyelination. The mixed axonal and demyelinating changes in findings. The environmental investigations disclosed that n-hexane was the provide an important diagnostic value in such cases.
