摘要
胸腔內畸胎瘤常見於前縱膈腔，極少見於肺內實質組織。一例16歲女性病患主訴半年來左胸疼痛及咳血，胸部X光及電腦斷層檢查發現在左肺上葉有一6×5×4公分囊狀腫瘤。手術後經病理診斷為良性肺內畸胎瘤。肺內畸胎瘤在英文文獻至1988年10月僅有27例報告。常見於左肺上葉，惡產腫瘤比例（34%）比前縱膈腔畸胎瘤（12─20%）較高，為其特點。

A 16 year-old female was diagnosed with intrapulmonary teratoma having had left chest pain for about half a year and hemoptysis in a more recent three weeks. A lobulated low density cystic tumor was found in the left upper lobe of her lung in chest X-ray and CT scan. After exploratory thoracotomy, the lesion was resected and was shown to be a mature cystic teratoma within the lung parenchyma. A teratoma consists of remnants of the three germ layers- endoderm, mesoderm and ectoderm. Although teratomata are commonly mediastinal neoplasma, they rarely occur within the lung parenchyma. Only 27 cases have been reported in English literatures prior to Oct. 1988. their origin remains unclear and it is postulated to be “thymic rests”. Intrapulmonary teratomata have most often been found in the left upper lobe and pancreatic tissue has also been commonly found in these lesions, as in the case reported here. The malignant incidence in the intrapulmonary teratoma (34%) is higher than the teratoma in the mediastinum (12-20%). The clinical symptoms are mostly nonspecific except expectoration of contents within the teratoma in about 10% of patients. The management of the mature intrapulmonary teratoma is by complete excision. Postoperative radiotherapy and chemotherapy may also be indicated in unresectable malignant intrapulmonary teratoma. To the authors knowledge, this is the first case of intrapulmonary benign teratoma reported in Taiwan.
