血中Somatomedin-C在嬰幼兒時期營養狀態之評估-特別與前白蛋白,胰島素及生長素之比較

Somatomedin-C in Infantile Nutritional Assessment with Comparison with Prealbumin, Insulin and Growth Hormone

摘要


所謂體介素-C(Somatomedin-C,簡Sm-C),由於具有類似胰島素的作用,而且又是生長素行使同化作用的媒介質,因此又稱為Insulin-Like Growth Factor I(簡稱GF-I);它主要是在肝細胞內被合成。在營養不良的成年病人,其血中Sm-C、胰島素和前白蛋白濃度會下降,但血中生長素濃度反而上升。在本研究中,我們研究年齡從1個月到1歲的嬰幼兒,針對以上四種肽肽的濃度,在反映營養不良之程度之營養不良,依照營養指數(nutritional index)分級,由正常組(n=37)到最嚴重的第三級營養不良組,血中Sm-C濃度(其濃度分別是,正常組:男361.2±158.9 mU/ml(n=18)(Mean±SD),女499.3±227.8 mU/ml(n=19);第一級營養不良組:256.9±81.1 mu/M1(n=19);第二級營養不良組:169.5±43.5 mU/ml(n=15);第三級營養不良組:76.2±26.5 mU/ml(n=18))與前白蛋白濃度(正常組:17.8±2.1 mg/dl (n=37);第一級營養不良組:14.2±3.0 mg/dl(n=17);第二級營養不良組:11.8±3.5 mg/dl (n=15)第三級營美不良組:9.7±2.4 mg/dl (n=18))最能依照營養不良程度加重而下降。因此我們的結論是,血中Sm-C值由於不會快速呈波動性變化,又不受睡眠影響,再加上有較短的半衰期,故可以用來當作營養狀態評估之每感指標。至於血中前白蛋白值如果沒有合併嚴重感疾病時,亦是很好的營養狀態之評估工具。

Somatomedin-C(Sm-C) 15 also called insul - in like growth factorl ( IGF-I ). It has insulin - like biological effects and serves as an anabol - ism - related mediator of growth hormone. Most Sm-C 15 synthesized in liver cells. There are de - creased blood levels of Sm-C, prealbumin and insulin, but increased blood levels of growth hormone in the adult witth malnutrition. We simultaneously measured the blood levels of the above four peptides in infants aged one month to one year. They were divided into four groups: normal, first class malnutrition, second class malnutrition and third class malnutrition . They were classified according to the Value of Nutrition Index. The utility of Sm-C measure - ment was compared to that of the measure - ments of prealbumin, insulin and growth ho - rmone in the nutritional assessment . The results revealed that the blood level of Sm-C decreased more in the cases of severe malnutrition ( Norm - al group : male 361.2士158.9 mU / ml ( n = 18 ) , female 499.3士227.8 mU / ml ;( n = 19 ) ; First class malnutrition group : 256.9士81.1 mUU / ml ( n = 17 ) ; Second class malnutrition group: 169.士43.5 mU / ml ( n = 15 ) ; And third class malnutrittion group : 76.2士26.5 mU / ml ( n = 18 ) ) . The blood level of prealbumin decreased more in the cases of severe malnuttrition ( Normal group : 17.5士2.1 mg / dl ( n = 37 ) ; First class malnuutrition:11.8士3.5 mg / dl ( n = 17 ) ; Second class malnutrition :11.8士3.5 mg / dl ( n = 15 ) ; And third class malnutrition : 9.7士2.4 mg / dl ( n = 18 ) ) . This study concluded that ( l ) the Sm-C level can serve as a sensitive indicator for malnutrition because it does not manifest rapid fluctuation and 15 not influenced by sleeping ; ( 2 ) the prealbumin level 15 also a good indicator for malnutrition state , after excluding severe infection .
